4. Orders invoiced only through authorized Hu-Friedy dealers.
5. Minimum purchase requirements must be met per product category.
6. Practitioner must include practicing state license number on the HUSAVE order form. |
7. You may cancel or modify your HUSAVE order at any time in writing by contacting Hu-Friedy. There is no need for annual renewal. You will continue to receive product as scheduled without interruption until cancelled or modified.
8. Hu-Friedy reserves the right to modify or cancel the HUSAVE program at any time.
9. Orders shipped F.O.B. Chicago are subject to shipping charges.
